Role Description

This is a full-time hybrid role for a Clinical Care Manager, located in Hobart, TAS, with some work from home flexibility. The Clinical Care Manager is responsible for overseeing and managing in-home care services, conducting comprehensive assessments, developing care plans, and coordinating with healthcare providers. Day-to-day tasks include case management, utilization management, and providing support to ensure the highest quality of care for clients. The role requires collaboration with team members and clients to monitor and adjust care plans as needed.

Qualifications

  • Social Work and Case Management skills
  • Experience in Utilization Management and Behavioral Health
  • Knowledge of Home Care practices
  • Strong organizational and communication skills
  • Ability to work independently and in a team environment
  • Bachelor's degree in Social Work, Nursing, or related field
  • Previous experience in a clinical care or management role is a plus
