Spiritual Care Specialist

This role provides spiritual care, leadership and support to clients, their families, team members and pastoral care volunteers.
The Spiritual Care Specialist visits clients in their own homes across the Brisbane region, requiring access to a car (mileage reimbursed) and working 3 days per week based out of Brisbane.
- Assessing client/family's spiritual and emotional needs and developing a spiritual care plan.
- Providing spiritual and emotional support to clients and their families.
- Liaising with relevant religious/spiritual leaders to coordinate provision of spiritual support.
About You:
- A commitment to engage and align with HammondCare's Christian Mission.
- Strong understanding and experience of people living at home with life-limiting illness including dementia, the impact on carers, and journeys of grief and bereavement.
- Strong personal and active Christian faith and church involvement.
Key Responsibilities:
- Spiritual Support: Provide ongoing spiritual support to clients and their families.
- Leadership: Lead and manage a team of pastoral care volunteers.
- Care Planning: Develop and implement individualized care plans for clients.
Requirements:
- Qualifications: A strong personal and active Christian faith and church involvement.
- Experience: Strong understanding and experience of people living at home with life-limiting illness including dementia.
- Skills: Excellent communication and interpersonal skills.
