
Chronic Condition Care Specialist

Job Overview
This role is a key part of our organization's efforts to provide high-quality patient care. The Chronic Care Coordinator will be responsible for developing and implementing individualized care plans for patients with chronic conditions.
Main Responsibilities:
- Deliver chronic condition management, health coaching, and health literacy services to patients.
- Develop strong relationships with patients, carers, families, and other healthcare professionals to ensure seamless care coordination.
- Provide education, support, and guidance to patients and their carers to promote healthy behaviors and self-management.
- Manage time effectively and prioritize tasks to meet deadlines and deliver results.
- Work autonomously and collaboratively as part of a team to contribute to organizational objectives.
Requirements:
- Relevant qualification in nursing or Allied Health with demonstrated experience in the field of expertise.
- Proficient in technology and digitally augmented practice, ideally with extensive telehealth experience.
- Awareness of AHPRA telehealth guidelines.
Working Arrangements:
This is a full-time permanent opportunity working from home. You will be required to provide evidence of vaccination against COVID-19, including booster, or evidence of a medical exemption.
