Job Opportunity: Delivering Chronic Disease Management and Prevention Services

The Integrated Primary Care (IPC) program focuses on ensuring access to allied health services in the primary health care space across the Yorke Peninsula, parts of the Mid and Lower North, Southern Fleurieu Peninsula, and Kangaroo Island.

  • This role will work as part of a specialist multidisciplinary allied health team to support the physical health of people with chronic medical conditions and risk factors for disease.
  • The Dietitian will be responsible for delivering chronic disease management and prevention services for clients accessing the IPC program.
Experience Requirements:
  • Demonstrated experience working within a fast-paced environment.
  • Proven experience of practice in community settings.
  • Ability to develop and implement procedures.
  • Capacity to develop resources for distribution to various stakeholders.
Qualification and Other Requirements:
  • Valid driver's licence.
  • Current or willing to obtain a Working with Children's Check (WWCC).
  • At least a degree-level qualification in Nutrition and Dietetics (graduate or post-graduate).
  • Current membership with the Dietitians Association of Australia, including Accredited Practising Dietitian status.
