Support Facilitator Job Summary

This position plays a crucial role in delivering high-quality support services to individuals with disabilities.

About the Role

We are seeking an experienced Support Facilitator to join our team. The successful candidate will be responsible for organizing and coordinating support services, ensuring that clients receive the best possible care.

The ideal candidate will possess strong organizational skills, the ability to work autonomously and as part of a team, and excellent interpersonal skills.

  • Deliver case management support to participants.
  • Making referrals to services to address participant concerns and coordinate care as part of a multidisciplinary team.
  • Manage a caseload through the provision of services in participants' homes.
  • Ensure the service exceeds all accreditation standards.
  • Maintain accurate and timely case notes and reporting on participants' progress.
  • Provide effective leadership to employees under your supervision.
  • Ensure rostering and allocation of staff is actioned in accordance to our Collective Agreement.
  • Participate in an On Call Rotation for emergency and shift management.
Key Skills and Qualifications

To be successful in this role, you will require:

  • Demonstrated knowledge of the National Disability Insurance Scheme and its impact on participants.
  • Strong organisation skills to effectively manage workload including complex rostering, caseload management and ensuring participant satisfaction.
  • Ability to work autonomously and as part of a team.
  • High level of interpersonal skills including developing and maintaining professional relationships within a team, with participants, across service providers and with various stakeholders.
  • Professional and compassionate verbal and written communication skills.
  • Demonstrated experience supporting participants and managing teams for those with complex medical needs.
  • High level of problem-solving skills to successfully identify problems, develop solutions and implement them using a logical and systematic approach.

Requirements

To apply for this role, you must possess:

  • A current Blue and Yellow Card.
  • An NDIS Worker Orientation Certificate.
  • A valid driver's licence.
  • First Aid and CPR certification.
  • A Flu Vaccination.
  • Covid Vaccination including a booster.
  • Relevant tertiary qualifications in Social Work, Allied Health or other relevant field.
  • Relevant experience in Case Management is highly regarded but not essential.
