About this role:

We are seeking an experienced and compassionate Support Coordinator to empower individuals with disabilities.

This is a dynamic opportunity to work closely with participants, their families, and service providers to ensure they receive the support and services needed.

Your primary responsibilities will include:

  • Assisting participants in understanding and implementing their plans.
  • Coordinating and connecting participants with appropriate service providers and community resources.
  • Supporting participants in building their capacity to independently manage their supports.
  • Maintaining accurate records and documentation.
  • Collaborating with participants, families, and other stakeholders to ensure a seamless service experience.

About you:

To be successful in this role, you will have:

A strong understanding of the NDIS and experience in support coordination or case management.

Excellent interpersonal and communication skills to engage effectively with participants and stakeholders.

Strong problem-solving skills and the ability to work independently.

Ability to multitask and manage time efficiently in a fast-paced environment.

A compassionate, professional, and dedicated approach to participant support.

Relevant qualifications in Social Work, Disability, Community Services, or a related field (desired but not essential).

Minimum 1-2 years of experience in NDIS support coordination, case management, or a similar role.

Experience working with people with disabilities or mental health conditions.

Knowledge of local disability support services and community resources.
