We are seeking a dedicated and compassionate Registered Nurse to provide exceptional care to our clients in the comfort of their own homes. As a key member of our team, you will be responsible for delivering high-quality, client-focused care in a community setting.
Key Responsibilities:
- Clinical in-home services include care plan assessment, medication management, incontinence care, wound management, chronic disease management, complex wound management, palliative care, and health and diabetes management.
- Clinical delivery and supervision of non-clinical team members in a community setting.
- Intake processing, clinical care planning, and reporting.
- Conducting client care assessments with a focus on Consumer Directed Care.
- Coordinate the provision of quality person-centred care according to the client's needs, service/care plans, and available funding.
- General administration for documentation relating to client care and in alignment with AHPRA regulations and legislation.
- On-call roster.
Requirements:
- Qualified Registered Nurse.
- Current AHPRA registration.
- First Aid & CPR certification.
- Australian National Police check.
- Professional Indemnity Insurance.
- Experience in assessing and planning care needs within the community.
- Valid Driver's Licence and reliable presentable vehicle.
- A smart mobile phone (iPhone / Android).
Preferred Skills:
- A passion for high-quality client-focused care in a community setting.
- Cert IV in Training and Assessment - highly desirable.
- Exceptional communication skills.
- Able to work autonomously or within a team environment.
- Excellent time management and organisation skills.
