Client Services Coordinator

The Client Service Coordinator is a key position within our organisation, responsible for delivering exceptional care and support to our clients. Key Responsibilities- Provide effective coordination of client care services, ensuring seamless transitions and continuity of care.
- Develop and maintain strong relationships with clients, families, and healthcare professionals to ensure optimal care outcomes.
- Collaborate with the General Manager Client Services to develop and implement operational initiatives that drive business growth and improvement.
- Contribute to the development and implementation of care plans that meet the unique needs of each client.
- Provide education and support to clients and their families on care-related topics and services.
- Proven ability to communicate effectively with diverse stakeholders, including clients, families, and healthcare professionals.
- Strong interpersonal and relationship-building skills, with the ability to build trust and rapport with clients and their families.
- Ability to work collaboratively as part of a team, with a focus on delivering exceptional client outcomes.
- High level of organisational and time management skills, with the ability to prioritise tasks and manage multiple projects simultaneously.
- Ability to maintain confidentiality and handle sensitive information with discretion.
