Job Title:

Seeking a dedicated and compassionate Disability Support Worker to provide high-quality care to individuals with complex needs.

Job Description:

The successful candidate will work closely with medical teams to deliver patient-centred care in a supportive environment. Responsibilities include delivering psychosocial assessments, leading complex discharge planning, and providing support to patients, families, and carers.

Key responsibilities include:

  • Deliver psychosocial assessments for oncology inpatients
  • Lead complex discharge planning with multidisciplinary teams
  • Provide support to patients, families, and carers
Requirements:

The ideal candidate will possess a minimum of 2+ years' experience as a disability support worker, with experience working with adults and children with high-intensity support needs and/or behaviours of concern.

Required qualifications and skills include:

  • At least 2+ years' experience as a disability support worker
  • Experience with high-intensity support needs and/or behaviours of concern
  • Current NDIS Worker Screening Check, Working with Children Check, National Police Check
  • Vaccinations (including COVID), CPR & First Aid
  • Right to work in Australia, 100 Points of ID, Driving Licence, reliable car
  • Cert III Individual Supports, Cert IV Disability and/or Mental Health Care qualification (preferred)
