We are seeking a highly skilled and experienced Support Coordinator to join our team. As a Support Coordinator, you will play a vital role in assisting Participants with complex needs to build personal capacity and make decisions about their supports.
This includes coordinating services, developing action plans, and liaising with stakeholders to ensure the best possible outcomes for our clients. You will work closely with our team to deliver high-quality support that meets the individual needs of each Participant.
Key responsibilities will include:
- Support coordination to implement NDIS plans
- Coordinate services and manage multi-disciplinary teams
- Provide advice and consultation on complex situations
- Develop action plans to resolve barriers arising from complexity
- Liaise and collaborate with key stakeholders
Our ideal candidate will have:
- 1+ years' experience in quality care services and implementing NDIS plans
- Relevant tertiary qualifications and understanding of the NDIS and Disability Act
- Strong interpersonal, negotiation, and complaints resolution skills
- Networking and relationship management skills
- Sound understanding of SDA
Additionally, you will require:
- NDIS Worker Screening Clearance
- Working with Children Check (Employee type)
- Valid Victorian Driver's Licence
