Key Responsibilities- Provide comprehensive assessment, diagnosis, treatment, complex discharge planning, and advocacy for clients across diverse programs.
- Ensure seamless client transitions through the healthcare system, liaising with staff and community agencies for continuity of care.
- Participate actively in ward rounds, case conferences, and family meetings.
- Maintain high standards of clinical activities, meeting competency requirements and supporting evidence-based practice.
- Facilitate effective communication within the organization and with external parties, ensuring accurate documentation and completion of statistical records.
- Contribute to workforce development and quality improvement initiatives in collaboration with the Clinical Manager.
- Recent experience in acute, subacute, and/or community hospital programs is essential.
- A commitment to providing excellent patient care across different settings is required.
