Community Rehabilitation Specialist

We are seeking a dedicated Rehabilitation Professional to join our dynamic Community Rehabilitation Team.
About the RoleThis multidisciplinary team provides clinical case management services to clients with complex needs, including those with acquired brain injuries and neurological conditions.
- Developing comprehensive assessments and rehabilitation plans
- Supporting clients in achieving their goals through tailored interventions
- Collaborating with stakeholders to coordinate services and referrals for clients
To be successful in this role, you will have expertise in neurological conditions and experience supporting clients through the rehabilitation process. A background in social work or occupational therapy would be highly regarded.
