NDIS Support Role

We are seeking a permanent NDIS Support Coordinator to assist Participants with complex needs to build personal capacity and make decisions about their NDIS plan.

A key responsibility of this role is to address barriers and reduce complexity in the support environment by supporting our Participants with:

  • Coordination of services to implement their NDIS plan.
  • Management of multi-disciplinary teams.
  • Advice and consultation with Participants, their families, and other stakeholders in response to challenging situations.
  • Development of action plans to resolve barriers arising from complexity.
  • Liaison and collaboration with stakeholders to establish goals for coordinating service delivery.

Successful applicants will have experience providing quality care services, implementing NDIS plans and building rapport with Participants in an outcome driven position within the Disability, Youth Welfare, Child Protection and community services sector.
