Family Services Practitioner Opportunity

This is a permanent role in a collaborative and supportive environment. As a Family Services Practitioner, you will be part of a multidisciplinary team delivering effective family-centred services.

The position provides holistic supports to infants, children and families. You will contribute meaningfully to the lives of our clients and communities by facilitating supportive connections.

In this rewarding opportunity, your key responsibilities include:

  • Empowering clients to recognise their strengths, aspirations and wellbeing goals and facilitate supportive connections that will help them achieve these.
  • Providing secondary consultations to family services and universal services.
  • Building the capacity of families to respond to their child's needs through coaching, mentoring and information sharing.
  • Making timely referrals to appropriate support and specialist services identified in the care plan.
  • Developing and implementing a shared and coordinated approach that incorporates social, emotional and physical determinants of health through collaboration.
  • Contributing to the improvement of service delivery by informing stand-up meetings and service design aligned to key performance indicators.
  • Contributing to program content and activities which support the wellbeing of infant, children and families.

To succeed in this role, you will need:

  • Qualification in social work or relevant field.
  • Minimum 4 years' experience working in partnership with families who have children aged 0-11 years.
  • Excellent motivational interviewing and holistic wellbeing assessment skills.
  • Strong communication skills with the ability to liaise with a wide range of stakeholders to create and maintain effective partnerships which generate improved outcomes for families.
  • Demonstrated ability to work independently and innovatively as well as collaborating effectively within a team environment.
  • A current Victorian driver's licence and Working with Children's Check.
