Job Title: Care Coordinators

We are seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will be responsible for providing support and assistance to clients in their homes.

Your key responsibilities will include:

  • Building strong relationships with clients and their families\
  • Conducting comprehensive assessments and developing personalized care plans\
  • Providing professional advice and direction to home care workers\
  • Liaising with clients and their families on a regular basis\
  • Maintaining accurate records and information\
  • E nsuring all practices are consistent with policy and legislative frameworks\

To be successful in this role, you will need:

  • \
  • A formal tertiary qualification in nursing or a related field\
  • Current registration as an Enrolled or Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)\
  • Demonstrated knowledge of the aged care sector\
  • A National criminal history/police check\
  • Industry applicable licenses (e.g. AHPRA registration, BC/NDIS)\
  • Valid First Aid and CPR certification\
  • Minimum of 2 COVID vaccinations\
  • An open driver's license and reliable transportation\
