Support Coordinator Role

We are seeking a skilled and compassionate Support Coordinator to join our team. The ideal candidate will have experience in assisting participants in understanding and implementing their NDIS plans, developing individualized support plans tailored to participant goals and aspirations, and providing guidance and advice on available services and supports.

About the Role

This is an exciting opportunity for someone who is passionate about delivering high-quality support and care services to individuals with disabilities. As a Support Coordinator, you will work closely with clients to help them achieve their goals and navigate the NDIS with confidence.

Key Responsibilities
  • Assisting participants in understanding and implementing their NDIS plans
  • Developing individualized support plans tailored to participant goals and aspirations
  • Providing guidance and advice on available services and supports
Required Skills and Qualifications

To be successful in this role, you will need:

  • Proven experience as a Support Coordinator or in a similar NDIS-related role
  • A relevant qualification in Disability, Social Work, or a related field
  • Strong knowledge of the NDIS framework and participant-centered planning
  • Excellent communication, organizational, and problem-solving skills
