Health Care Navigator

Delivering goal-directed tele-health services, our team is seeking an experienced registered nurse (with AHPRA registration) to provide evidence-based interventions. Flexible working arrangements are available, including hybrid options.
We prevent and manage workplace injuries and illnesses with tailored solutions, believing that workforce health and wellbeing services benefit people and businesses alike.
Key ResponsibilitiesThe role includes:
- Supporting clients with empathy and expertise to improve their personal health and wellbeing goals.
- Coordinating supports and wellbeing services.
- Providing initial early intervention triage with clients, supporting both physical and mental health services.
- Managing case coordination activities, including liaising with stakeholders and creating documentation such as treatment plans.
Qualifications include:
- Registered nurse with full registration.
- Mental health support service experience is highly desirable.
- Caring and empathic nature.
- Experience in care coordination or occupational rehabilitation.
- Strong written and verbal communication skills.
- Good IT and time management skills.
- Strong customer service focus.
