We are seeking a dedicated professional to support men experiencing homelessness and facing complex challenges.
This is a permanent full-time position based in Flagstaff In West Melbourne and The Open Door in North Melbourne, VIC.
- Develop and implement case plans in partnership with clients.
- Support transitions to long-term housing through referrals and outreach.
- Advocate across housing, health, legal, and community systems to meet client needs.
Successful applicants will have experience in the social services sector, strong communication and advocacy skills, and a commitment to inclusive practice.
As a Case Manager, you will be responsible for leading warm referrals and coordinated outreach efforts. You will collaborate with clients to develop strengths-based case plans and provide individualised support.
The role requires a strong sense of purpose and empathy, as well as knowledge of homelessness, trauma, and housing systems. A degree qualification in Social Work, Psychology, or a related field is highly desirable.
