Job Role:

We are seeking a dedicated and compassionate professional to join our team as a Support Worker.

As a Support Worker, you will be responsible for providing person-centred care and support services to individuals with disabilities.

You will work collaboratively with customers, families, caregivers, and key stakeholders to develop and maintain professional relationships and networks.

Main Responsibilities:
  • Provide person-centred care to help customers achieve their goals and aspirations
  • Contribute information for the development of Customer support plans
  • Provide support to customers with personal care, living skills, transport, social activities, medication, and other support as required
  • Keep customer records up-to-date on a daily basis
Key Requirements:
  • Completed Cert 3 or 4 in disability, individual support, aged care, or nursing
  • Valid License and access to own vehicle (or willing to apply)
  • NDIS Worker Screening Check (or willing to apply)
  • At least 6 months' experience in disability, aged care, or nursing sector
  • Proof of Vaccination (COVID-19)
  • Working with Children's Check (or willing to apply)
