About the Position

We are seeking a dedicated Support Professional to work with individuals with disabilities, helping them achieve their goals and enhance their quality of life through person-centered care.

This role offers the opportunity to make a significant impact on the lives of individuals and their families. As a Support Professional, you will work closely with clients, their families, and a multidisciplinary team to ensure high-quality support services are delivered in alignment with NDIS framework standards.

Key Responsibilities:

  • Client Support & Advocacy: Serve as the primary point of contact for clients, providing guidance and support in all aspects of their care and advocating for their needs.
  • Care Coordination: Develop and implement tailored care plans that align with each client's goals and NDIS funding requirements.
  • Family & Stakeholder Engagement: Work closely with families, caregivers, and other professionals to ensure consistent, holistic support for clients.
  • Progress Monitoring & Reporting: Track client progress, make adjustments to care plans as necessary, and ensure accurate documentation in line with NDIS standards.
  • Training & Support: Provide training and support to families, carers, and team members in implementing behavior support strategies and other necessary interventions.

Essential Skills & Qualifications:

  • Relevant Qualifications: A degree in Social Work, Psychology, Occupational Therapy, or a related field is desirable.
  • Experience: Experience working with individuals with disabilities, ideally within the NDIS framework.
  • Communication Skills: Strong interpersonal skills and the ability to engage effectively with clients, families, and team members are essential.
  • Knowledge of NDIS: Familiarity with NDIS policies, procedures, and individualized care approaches is required.
  • Problem-Solving Abilities: Capacity to develop innovative solutions to support individuals in achieving their goals.
  • Team Player: Collaborative attitude and ability to work in a multidisciplinary team is vital.
