About the Role

The Case Worker role delivers timely participant-centered intake and case management for individuals requiring crisis accommodation support at Crerar Road. Working collaboratively with a multi-disciplinary team, the Case Worker supports housing stability and wellbeing by facilitating referrals and encouraging participation in site activities.

Key responsibilities include:

  • Providing timely and effective case management services to participants
  • Developing and implementing individualized plans to achieve participant goals
  • Collaborating with internal stakeholders to ensure seamless service delivery
  • Facilitating referrals to relevant services and agencies as required
What We Are Looking For

To be successful in this role, you will possess the following skills and qualifications:

  • A relevant tertiary qualification (Certificate III or above) in social work, community services, welfare, or a related field
  • Strong case management, assessment, and planning skills
  • Effective communication, culturally responsive practice, and advocacy skills
  • Demonstrated ability to work collaboratively in a diverse, multi-disciplinary team
