We are seeking a highly skilled Dementia Care Professional to join our team. This role offers an exciting opportunity to make a meaningful impact in the lives of individuals and families affected by dementia.
About the RoleThis is a permanent full-time position, offering a chance to work with clients and their loved ones to develop personalized care plans, identify potential risks, and provide comprehensive support services.
- Conduct thorough assessments to identify client needs and develop tailored care plans that address their physical, emotional, and social requirements.
- Work collaboratively with healthcare professionals, family members, and caregivers to ensure seamless coordination of care and promote optimal outcomes for clients.
- Utilize evidence-based practices to deliver high-quality care, monitor progress, and adjust plans as needed to reflect changing client needs.
- Provide guidance and support to clients and their families, empowering them to navigate the healthcare system and access relevant resources and services.
To be successful in this role, you will need:
- A formal qualification in Social Work, Allied Health, Nursing, or a related field;
- Eligibility for membership of the AASW and evidence of current CPE, as per AASW requirements, or equivalent registration;
- Demonstrated commitment to delivering exceptional client service, staying up-to-date with industry developments, and prioritizing person-centered care and evidence-based practice.
