The role of Clinical Partner involves delivering high-quality clinical care that prioritizes clients' unique needs and preferences.

Key Responsibilities:
  • Assess client needs and develop/review care plans to ensure optimal outcomes.
  • Provide evidence-based, best-practice clinical care holistically, ensuring client goals and needs are understood and addressed.
  • Collaborate with the care management team to achieve optimal outcomes for clients in a community setting.
  • Guide care staff to ensure safe and continuous delivery of high-quality clinical care.

The ideal candidate will possess a nursing qualification, current registration with AHPRA as a Registered Nurse (Division 1, General), and excellent communication and collaboration skills.

A passion for client-focused service delivery in aged and community/home care settings is essential. The ability to work effectively in a multidisciplinary team and independently across multiple sites is also required.

Additional requirements include a current driver's license and willingness to travel between HammondCare sites and services.

Commitment to professional development, empathy, patience, and a passion for delivering high-quality care is also essential.
