Our goal is to provide the highest standard of care for our clients. This role involves delivering holistic, person-centred care to older adults experiencing health challenges, chronic conditions, and complex needs.

About the Role

This is a meaningful opportunity to make a real difference in the lives of older people—helping them stay well, stay safe, and stay connected. As a Community Nurse, you will be working as part of a multidisciplinary team to provide early intervention, health education, and recovery-focused support within clients' homes and community settings.

Responsibilities
  • Providing direct clinical nursing care to our clients in their homes in line with best practice nursing procedures
  • Conducting clinical assessments, dressings, medication assessment and catheter changing
  • Developing a holistic nursing care plan in collaboration with client, carer, Care Advisor and Allied health where relevant
  • Providing safe, quality nursing practice to clients with specific needs and/or complex needs eg. Wound care, palliative care, education and support
  • Liaising with all parties involved in the care of the client to maintain holistic health care and wellbeing
Requirements

To be successful in this role you'll need:

  • A genuine passion for providing high-quality care to the elderly
  • Current AHPRA registration following completion of a Bachelor of Nursing or similar
  • Minimum 3 years postgraduate nursing experience
  • Experience in clinical assessments
  • Initiative and the ability to work independently
  • Strong computer skills especially Microsoft Office products and ability to pick up new systems
  • NSW Drivers license and a reliable vehicle
