
Holistic Community Care Specialist

We are seeking an experienced Restorative Care Specialist to join our team. The successful candidate will play a key role in the clinical coordination and delivery of restorative and rehabilitative care services in accordance with a client's support plan.
The role ensures optimal health outcomes for clients while promoting independence and functional recovery, and ensures culturally responsive, holistic, and inclusive care is provided.
Key Responsibilities:- Community Engagement: Establish strong relationships with community stakeholders and promote our services to potential clients.
- Assessment and Restorative Care Coordination: Conduct comprehensive assessments of clients' needs and develop individualized care plans to ensure the best possible outcomes.
- Ongoing Support and Guidance: Provide regular support and guidance to clients and their families to ensure they receive the care and assistance they need.
- Quality Care Processes and Risk Management: Develop and implement quality care processes and risk management strategies to ensure the highest level of care is delivered.
To be successful in this role you will have:
- Current Registration: Current registration as a Registered Nurse or Allied Health professional.
- Specialist Certifications: Specialist certifications / qualifications in relevant disciplines are highly regarded.
- Extensive Experience: Extensive experience in clinical coordination, case management, or restorative care within aged care, disability, or community services.
- Strong Understanding: Strong understanding of reablement, functional rehabilitation, and person-centred care models.
- Proficiency in Risk Management: Proficiency in risk management, compliance, and quality assurance processes.
- High-Level Communication Skills: High-level communication, problem-solving, and decision-making abilities.
- Exceptional Customer Service: Exceptional customer service, interpersonal and communication (listening, written and verbal), and liaison skills, with the ability to deliver dynamic, person-centred service within the community and in clients' homes.
- Competence in Clinical Documentation: Competence in using clinical documentation and care management systems.
- Ability to Build Relationships: Ability to build productive networks and relationship development.
- Initiative and Problem-Solving: An ability to take initiative, problem solve and make decisions.
- Operational Key Performance Indicators: Experience working to operational key performance indicators (KPIs) and achieving business development targets, in a performance-driven environment.
- Time Sensitive Tasks: Experience managing client budgets, being responsive and accountable for time sensitive tasks.
- Drivers Licence and Own Car: Drivers licence & own car.
