Job Summary:

We are seeking a highly skilled liaison professional to guarantee the delivery of high-quality care services that meet and exceed the expectations of our clients, customers, and stakeholders.

Main Responsibilities:

  • Provide personalized information about care services and collaborate with new and existing service users to review and plan their needs
  • Conduct comprehensive client intake assessments and develop tailored care plans in consultation with clients and their family members
  • Regularly review and adjust care plans based on changing client needs, goals, and preferences
  • Undertake phone assessments for new and existing clients based on referrals received
  • Complete all necessary documentation for intake and assessment protocols using prescribed tools and databases
  • Promote seamless and positive experiences for clients by providing expert advice on care planning and coordination

Key Requirements:

  • Demonstrated ability to assess, plan, and evaluate care with clients, family members, and other relevant individuals or groups
  • Strong understanding of community-based services and referral pathways
  • Excellent organizational and time management skills, including experience with scheduling, rostering, and reporting activities
  • Administrative experience in information sharing, report writing, and data entry
  • Ability to work independently and as part of a team, both on-site and off-site as required
