We are seeking a dedicated and compassionate professional to join our Home and Community team as a Team Leader and Client Liaison Officer.

**Job Description:**

In this exciting dual role, you will be responsible for providing guidance and support to our Home Care Workers and Support Workers while also managing Home Care Package clients. You will work closely with our teams to ensure that our clients receive high-quality care and services that enable them to maintain their independence.

**Key Responsibilities:**

  • Provide supervision, leadership and guidance to Home Care and Support Service workers
  • Assist with the orientation of new staff
  • Undertake assessments, support planning and package care coordination
  • Undertake case management activities
  • Support clients with their budgets

**Requirements:**

To be successful in this role, you will require:

  • Certificate IV in Home and Community Care or equivalent
  • Current National Police Certificate - or willingness to obtain
  • Current Drivers Licence and own reliable vehicle
