Job Title

Clinical Aged Care Assessment Officers are required to work within our team.


  • Client assessments and reviews will be undertaken to identify individual needs and develop tailored support plans.
  • Participation in clinical triage and case conferencing is essential for effective care coordination.
  • Risks associated with client care must be identified, monitored and managed.

Requirements

Applicants should possess:

  • Tertiary qualifications in a health-related discipline directly related to aged care or specialist areas such as Allied Health or Social Work.
  • Current registration with the relevant professional association is mandatory.
  • Experience working in aged care settings, preferably with at least one year's experience in needs assessment, is required.
