Job Overview

As a Clinical Aged Care Specialist, you will play a pivotal role in identifying the needs of older adults and connecting them with essential services that support their independence.

Key Responsibilities:
  1. Conduct comprehensive assessments to identify the needs of older adults, ensuring their optimal care and well-being.
  2. Facilitate referrals to relevant services that promote independent living, fostering strong partnerships between families, carers, and service providers.

Required Skills and Qualifications:
  • At least 1 year of experience in aged care, including client assessments and/or service delivery.

  • Current unrestricted General Registration with AHPRA or relevant professional association.
  • Proficiency in Microsoft Office applications, particularly Word, Excel, Outlook, and client management systems.
  • Current Australian driver's licence, comprehensively insured vehicle, and ability to travel within your designated service area.
