Support Role - NDIS Journey

This position plays a vital role in empowering individuals to navigate the National Disability Insurance Scheme (NDIS) journey, ensuring seamless access, effective plan development, and timely implementation of funded support.

The successful candidate will be instrumental in helping participants build confidence, independence, and informal support networks, ultimately enhancing their overall well-being.

Key Responsibilities:
  • Accompany participants through the entire NDIS journey, providing guidance and support every step of the way.
  • Collaborate with participants to identify and build on their strengths, fostering confidence and independence.
  • Ensure timely and high-quality service delivery, adhering to performance targets and exceeding expectations.
  • Establish and nurture partnerships with local organizations, participate in community events, and champion inclusion through grassroots engagement.
  • Utilize NDIS and Feros Care systems to document participant interactions, track progress, and pinpoint areas for improvement.
Requirements:
  • Experience supporting people with disabilities in community services, social work, or related settings.
  • In-depth understanding of the NDIS participant pathway and planning processes.
  • Proven ability to manage caseloads effectively, meeting service delivery targets and exceeding expectations.
  • Excellent communication, interpersonal, and problem-solving skills, with a strong emphasis on stakeholder relationship building.
  • Strong data entry, reporting, and case management system experience.
Qualifications & Checks:
  • Qualification in community services, disability, social work, or a related field.
  • Access to reliable transportation for local travel.
  • NDIS Workers Screening Check & Working with Children's Check (or willingness to obtain).
  • Undergo a Nationally Coordinated Criminal History Check.
  • Valid Australian Working Rights.
  • Lived experience of disability is highly valued.
