We are seeking an experienced Social Worker to join our multi-disciplinary team at Caulfield Hospital. The successful candidate will work collaboratively with colleagues to provide community-based health and wellbeing services to people across Melbourne's south-eastern suburbs.

- Part time (48 - 64 hours per fortnight, negotiable)
This role would suit a dynamic and compassionate Social Worker with a passion for working with older adults and their families. The ideal candidate will have experience in navigating pathways within the healthcare system and be able to respond effectively to issues affecting clients experiencing elder abuse or family violence.
The Social Worker will see clients either at the community health centre or in their own homes, providing advanced Social Work services including psychosocial assessment, counselling, interim case management and casework.
Key Responsibilities:
- Provide individual and group work, advocacy, and outreach services to older adults and their families.
- Navigate pathways within the healthcare system, including My Aged Care, NDIS, Commonwealth Home Support Program, Home Care Packages, and VCAT.
- Respond effectively to issues affecting clients experiencing elder abuse or family violence.
Qualifications and Experience Required:
- Tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Workers.
- At least three years' post-qualifying experience in a similar role.
- A current Victorian Driver's Licence.
