Job Role: Adult Social Worker

Our team in Locality is seeking an experienced and skilled social worker to join our busy Adults Community/Locality Team.

The successful candidate will work 36 hours per week, Monday to Friday, 9 am – 5 pm, with a hybrid working model that includes mandatory office days on Wednesday and Friday, plus training and duty days.

Key Responsibilities:

  • Completing comprehensive social care needs assessments for adults.
  • Developing and implementing support plans to meet individual needs.
  • Commissioning services to ensure the best possible outcomes for service users.
  • Conducting risk assessments and managing risks, including attending Multi-Agency Risk Assessment Conferences (MARACs).
  • Leading safeguarding initiatives and promoting a culture of safety within the team.
  • Applying Mental Capacity Act/Best Interests principles to ensure the rights and dignity of service users are respected.
  • Working with Continuing Healthcare teams to ensure seamless transitions and effective care planning.

Requirements:

  • Relevant qualifications and experience as a social worker.
  • Proven ability to work effectively in a team environment.
  • Excellent communication and interpersonal skills.
  • Ability to manage complex casework and prioritise tasks effectively.
  • Valid driving license and access to a car for work purposes.
