Key Responsibilities:

  • Provide personalized care and support to clients, promoting their independence and well-being.
  • Escalate client issues to a coordinator or on-call team member as needed.
  • Communicate effectively with clients, families, and advocates to ensure seamless care and support.
  • Engage in individual and group care activities to promote social interaction and community participation.
  • Manage time effectively to ensure timely delivery of care and support services.
  • Guide clients to their coordinator for service changes and updates.
  • Tailor support to meet the diverse needs of clients, including those from culturally diverse backgrounds.
  • Assist clients in communicating service changes and updates.
  • Monitor and report changes in clients' well-being to ensure timely interventions.

Requirements:

  • Previous experience in community home care or a related field.
  • Proven ability to communicate effectively with clients and stakeholders.
  • Experience in catering to culturally diverse clients – a plus.
  • Ability to work autonomously with minimal supervision.
  • Proficiency in understanding and following client care plans.
  • Track record of building rapport with clients and stakeholders.
  • Essential minimum qualification: Certificate III in Individual Support or Community Services.
