Aged Care Assessment Role
Making a meaningful difference in the lives of older adults requires dedication, clinical expertise, and a passion for delivering high-quality care. As a valued member of our team, you will play a crucial role in assessing and planning personalized care to meet the complex needs of vulnerable individuals.
Key Responsibilities:
- Conduct comprehensive assessments of older adults to identify their needs and develop effective care plans that prioritize health, wellbeing, and quality of life.
- Collaborate with multidisciplinary teams to deliver coordinated care and support services that cater to diverse cultural backgrounds.
- Utilize evidence-based practices and knowledge of My Aged Care programs to inform assessment and care planning decisions that promote best outcomes.
- Demonstrate a commitment to cultural safety and respect for diversity, particularly in relation to Aboriginal and Torres Strait Islander peoples, through inclusive practice and community engagement.
Requirements:
- Hold relevant tertiary qualifications as per clinical discipline (nursing, occupational therapy, social work, physiotherapy) with full registration or eligibility for membership of the relevant professional association.
- Possess a current NSW driver's license and be willing to travel between facilities to ensure continuity of care.
- Proven ability to work effectively in a fast-paced environment and adapt to changing priorities while maintaining a patient-focused approach.
