Comprehensive Aged Care Specialist

We are seeking a skilled and compassionate aged care specialist to join our team. As an aged care specialist, you will be working closely with older people to assess their care needs and develop personalized plans to support them in their daily lives.

Key Responsibilities:

  • Conduct thorough assessments of older individuals to identify their care requirements
  • Develop and implement customized care plans that cater to the unique needs of each person
  • Collaborate with multidisciplinary teams to ensure seamless care coordination
  • Provide education and support to individuals and their families on aging-related diseases and care options

Requirements:

  • Relevant tertiary qualifications in a clinical discipline (nursing, occupational therapy, social work, physiotherapy)
  • Full registration or eligibility for membership of relevant professional associations
  • Proven experience working in aged care settings, with a strong focus on assessment, care planning, and coordination
  • Demonstrated expertise in dementia and age-related disease assessment and management
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
