The Opportunity

Deliver tailored support to residents in their own homes through Home Care Packages (HCP), fostering independence and well-being.

As a key member of our team, you will assess care needs, provide guidance, and manage home care packages, working closely with residents to develop personalized care plans.

This rewarding role offers the chance to actively improve quality of life for residents while collaborating within a supportive team environment.

Key Responsibilities:

  • Meet with prospective residents and their families to understand and assess care needs
  • Provide information on care needs, services, and funding options available
  • Establish comprehensive home care plans aligned to resident care needs
  • Prepare and maintain records, documents, and reports as required
  • Deliver exceptional customer service to residents, families, staff, and external health professionals
  • Monitor the effectiveness of service provision by ensuring consumers receive the care and support services they require
  • Liaise effectively with external service providers who provide care within the community
  • Promote our services by attending community events and providing support throughout the My Aged Care journey

What You Will Bring:

  • Current registration with experience preferably in a Home or Community Care setting
  • Demonstrated experience in understanding and managing care plans and clinical assessments
  • Solid understanding of My Aged Care and Home Care Packages or similar commonwealth funding package models
  • Excellent people management, technology, and communication skills
  • Self-motivated individual with a practical and professional approach to case management
  • Current Drivers License, Police Check, CPR, and COVID Vaccinations up to date (required)
