Our Homelessness Service in Southport is seeking a dedicated professional to join our team as a Case Manager. This fixed-term, full-time role will make a real difference in the lives of individuals and families experiencing homelessness.

Your Key Responsibilities

As a Case Manager, you will work with clients to maintain emergency motel accommodation while helping them achieve their goals and find long-term housing solutions. Your key tasks will include:

  • Maintaining strong relationships with clients and stakeholders through effective communication and rapport-building skills.
  • Assessing client needs, strengths, safety, and risk to develop and implement a holistic case plan.
  • Delivering client-centred case management from a strengths-based and trauma-informed approach.

To succeed in this role, you will have:

  • Experience providing support to vulnerable persons in crisis.
  • A relevant qualification in a community services-related field, such as a Cert IV in Community Services.
  • An Australian Drivers Licence and QLD Working with Children Check - Blue Card.
