Job Title: Family Focused Service Coordinator

This role involves working closely with children, young people, their families and carers to establish and implement effective service plans that meet the unique needs of each individual.

  • You will build strong relationships with the children, young people and their families, ensuring their voices are heard and their needs are met.
  • In this position, you will also support and monitor placements, guiding carers in their foster care journey and helping them develop strategies that best support the children and young people in their care.
Main Responsibilities:
  • Collaborate with children, young people, their families and carers to create and implement tailored service plans.
  • Develop and maintain strong, functional relationships with both the children and their families.
  • Support and monitor placements, providing guidance to carers on how to best support the children and young people within their care.
Key Requirements:
  • A relevant bachelor's degree such as Social Work, Social Welfare, Psychology or a related field is required.
  • Previous experience in case management, particularly in an Out of Home Care (OOHC) /Child Protection setting.
  • A strong understanding of trauma and its effects on children and young people, as well as experience supporting those with disabilities.
  • Familiarity with the NSW Permanency Support Program is highly desirable.
