About the Role

This position involves working with individuals and families who are experiencing or at risk of homelessness.

As an Outreach Case Manager, you will provide comprehensive case management services to support clients in achieving housing stability, social inclusion, and stronger connections with community supports and services.

Your key objectives will be to deliver effective outreach case management, conduct comprehensive intake and assessments, develop and review individualised case plans, and uphold a client-centred approach that respects individual rights, cultural needs, and freedom from discrimination.

Key Responsibilities:
  • Deliver outreach case management services to diverse cohorts.
  • Conduct comprehensive intake and assessments using a trauma-informed and Housing First approach.
  • Develop and regularly review individualised case plans tailored to each participant's goals, background, and needs.
  • Uphold a flexible, inclusive, and client-centred approach.
Requirements:
  • Tertiary qualifications (minimum Diploma) in Community Services, Social Work, or a related field.
  • Previous experience in the homelessness sector is highly desirable.
  • A current paid NSW Working With Children Check.
  • National Police Check (or willingness to obtain).
