**Job Title:** Senior Disability Support Specialist

 

We are seeking a highly skilled and dedicated Senior Disability Support Specialist to join our team. This role will involve working with individuals living with disabilities, providing tailored support and services to enhance their quality of life, independence and community inclusion.

 

**Responsibilities:
• Ensuring that clients receive the best possible care, considering their goals and aspirations.
• Supporting individuals to formulate their goals and aspirations prior to NDIS planning meetings.
• Participating in team meetings and case conferences.
• Maintaining accurate and timely records, adhering to relevant policies and procedures.
• Collaborating with clients and stakeholders to develop care plans and agreements.
• Providing person-centred services, tailored to meet individual requirements.
• Enhancing client opportunities for improved quality of life, independence and community inclusion.
• Monitoring and reviewing services to ensure they meet client needs and expectations.

 

**Required Skills and Qualifications:

• Qualifications in Community Services or relevant demonstrated experience.
• Excellent administrative, time management and organisational skills.
• Experience working within the Disability sector, including knowledge of NDIS and DHS standards.
• Understanding of up-to-date Disability related legislation, policy and processes.
• Ability to work collaboratively and influence stakeholders at different levels.
• High degree of professionalism, optimism and resilience.
• Excellent interpersonal and communication skills.
