Case Manager Role Overview

Our organisation is seeking a dedicated and compassionate Case Manager to work with Aboriginal and/or Torres Strait Islander men who are at risk of homelessness.

This role focuses on providing tailored case management to help men overcome challenges and find stable housing solutions. The successful candidate will be instrumental in empowering these men to build better futures through social inclusion, community engagement, and access to support services.

Key Responsibilities:

  • Providing client-centred case management support tailored specifically to the needs of Aboriginal and/or Torres Strait Islander men.
  • Leading and facilitating programs focused on the unique needs of men.
  • Developing and maintaining individualised case plans that align with each client's background, goals, and evolving needs.
  • Ensuring all interactions are culturally safe and non-discriminatory, prioritising respect, dignity, and individual rights in your approach.

Requirements:

  • Tertiary qualifications within community services, social work or related discipline (Minimum Certificate IV, Diploma, or working toward a degree).
  • A commitment to empowering men to build independence, resilience, and stronger community connections.
  • Knowledge of services and supports that specifically address the needs of men affected by disadvantage and complex needs.
  • Solid level of experience (minimum 2+ years) in case management and systems for disadvantaged and/or complex needs clients.
