Clinical Partner

HammondCare at Home is seeking an experienced clinical professional to deliver high-quality, person-centered care in the community setting.

The role provides evidence-based clinical care that supports individuals to live well in their own home, with a focus on assessing clinical needs and developing tailored care plans.

This position works collaboratively with a multidisciplinary team to ensure excellent patient outcomes and provides guidance to care staff on safe and continuous delivery of high-quality care.

Key responsibilities include:

  • Delivering high-quality, person-centered clinical care to clients in the community setting.
  • Assessing clinical needs and developing tailored care plans in partnership with clients and key stakeholders.
  • Working collaboratively with the care management team to ensure best outcomes for clients.
  • Providing guidance and support to care staff on safe and continuous delivery of high-quality care.
Required Skills and Qualifications
  • Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General).
  • Passion for and previous experience in client-focused service delivery in aged and community/home care settings.
  • Excellent communication and collaboration skills, with a commitment to advocacy and person-centred care.
  • Ability to work effectively in a multidisciplinary team and independently across multiple sites.
  • Current driver's license and ability to travel between sites and services.
  • Excellent administrative and computer skills, including digital health records and care coordination tools.
  • Commitment to professional development.
  • Empathy, patience, and a passion for delivering high-quality, person-centred care.
Desirable Requirements
  • Nursing experience in aged care and community care.
  • Ability to identify and implement innovations in care delivery and service improvement.
