About This Role

We are seeking a qualified and compassionate professional to join our Community Allied Health Team. As a Social Worker, you will be part of an interdisciplinary team delivering holistic, client-focused care to individuals in their homes, clinics, aged care facilities, and other community-based settings.

You will provide comprehensive assessments and interventions for patients and carers, deliver case planning, referral, and advocacy services, and work collaboratively with teams to support integrated care delivery.

The successful candidate will have a Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers (AASW) and a well-developed understanding of psycho-social impacts of acute and chronic health conditions and related social work interventions required in community practice.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments and develop appropriate intervention plans
  • Deliver case planning, referral, and advocacy services to support individuals and families
  • Work collaboratively with interdisciplinary teams to achieve integrated care outcomes
  • Develop and maintain effective relationships with stakeholders, including clients, families, and healthcare professionals

Required Qualifications and Skills

  • Degree in Social Work from an accredited course
  • Australian Association of Social Workers (AASW) membership eligibility
  • Well-developed understanding of psycho-social impacts of acute and chronic health conditions and related social work interventions
  • Effective communication and interpersonal skills
  • Ability to work collaboratively within a team environment
