Job Summary:
This is a Permanent Part Time position requiring an individual with exceptional communication and interpersonal skills who is passionate about delivering care coordination using a holistic approach.
As a Care Coordinator, you will collaborate with key stakeholders to ensure people with chronic conditions receive suitable services to attain their health goals. Your responsibilities include:
- Developing a network to provide individualised care for individuals with complex medical and psychosocial conditions.
- Coordinating a care service across organisational boundaries integrated with primary care, community, acute, and social support services.
- Ensuring people with chronic/complex conditions receive optimal care and support to remain safely in the community.
- Contributing actively to the Care Team to ensure equitable access to a flexible, supportive district-wide service.
