We are currently seeking an experienced Care Manager to join our team on the Gold Coast. As part of our dynamic and empathetic clinical team, you will play a vital role in ensuring the highest standard of ongoing assessment, care planning and evaluation for residents, helping them to live longer, healthier, and happier lives.

Duties include:
  • Overseeing the daily clinical operations of the care home
  • Supporting and leading the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education
  • Reviewing clinical incidents, complaints, and changes
  • Leading compliance with Aged Care Accreditation Standards and Outcomes
  • Managing person-centred care planning and evaluation for residents - including case conferencing with families and carers
  • Preparing and conducting clinical governance audits
  • Overseeing medication management in accordance with resident needs and legislative requirements
  • Liaising with allied health, medical practitioners, and other services to provide optimal resident care
Requirements:
  • Bachelor of Nursing with current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Post-graduate qualification in aged care highly regarded
  • Experience managing a minimum of 30 beds
  • Proven track record in delivering quality clinical care within an aged care or acute setting
  • Excellent clinical assessment skills and care planning capabilities
  • Highly effective communication, leadership, and relationship management skills
