Care Manager Position

We are seeking a skilled and compassionate Care Manager to lead our clinical team in delivering exceptional person-centered care for residents.

Key Responsibilities:

  • Oversee daily clinical operations of the care home, ensuring high-quality resident care.
  • Serve as a mentor and coach to the care home clinical team, providing guidance on staff allocations and delegation.
  • Review and analyze clinical incidents, complaints, and changes to improve processes and outcomes.
  • Lead compliance with assessment requirements, working closely with relevant stakeholders.
  • Develop and implement person-centered care plans, conducting case conferencing with families and carers as needed.
  • Prepare and conduct clinical governance audits to ensure adherence to standards.
  • Manage medication administration in accordance with resident needs and legislative requirements.
  • Liaise with allied health professionals, medical practitioners, and other services to optimize resident care.

Requirements:

  • Bachelor of Nursing degree with current registration
  • Postgraduate qualification in aged care (desired)
  • Minimum 30 beds management experience
  • Proven track record in delivering quality clinical care within an aged care or acute setting
  • Excellent clinical assessment skills and care planning capabilities
  • Highly effective communication, leadership, and relationship management skills
  • Familiarity with Resident Rights, Aged Care Accreditation Standards, and Outcomes
