Role Overview

We are currently seeking a dedicated professional to join our team as a Case Manager. This role is an exciting opportunity to make a positive impact in the lives of children and young people.

About the Role

The Case Manager will be responsible for providing case management to children and young people in out-of-home care arrangements. The successful candidate will have excellent communication skills, with the ability to build strong relationships with children, young people, and their carers.

  • Develop and implement individualized care plans that meet the unique needs of each child or young person.
  • Collaborate with the Department of Families, Fairness and Housing to ensure that care arrangements are provided in a safe and nurturing environment.
  • Provide regular support and coaching to foster carers to help them provide a secure and stable environment for the children and young people in their care.

Key Responsibilities

The Case Manager will be responsible for:

  • Conducting home and outreach visits to assess the needs of children and young people and develop tailored support plans.
  • Working collaboratively with other professionals to ensure that the health, development, and wellbeing needs of children and young people are met.
  • Providing written reports and case notes to high standard.

Requirements

To be successful in this role, you will need:

  • Bachelor level qualifications in Social Work, Social Science, Psychology or Human Services.
  • Knowledge of trauma-informed care and responses to pain-based behaviors.
  • Experience providing case management to children and young people, preferably within the child protection context.
