As a highly skilled nursing professional, you will play a pivotal role in delivering exceptional care to our patients. Our Acute Care of the Elderly (ACE) model is based in Ward G3 at a hospital where we provide 24/7 General Medical care for older persons with acute care needs and geriatric syndromes.
The RoleYou will have the opportunity to work closely with multidisciplinary teams to enhance independence and functional ability for our patients. As a clinical nurse, your practice will lead to excellence in patient care, focusing on establishing relationships between patients and yourself to achieve the best possible outcomes.
Key Responsibilities- Establishing and evaluating patient plans of care in partnership with patients, carers, and other healthcare team members
- Proven people and communication skills, including relationship building, cooperation, conflict resolution, influencing others, and facilitating open discussions
- Patient-centred focus, adaptability, respecting diversity, innovative thinking, application of evidence into practice, and self-management
- Registered as a qualified nurse with the relevant governing body
- Current and satisfactory police clearance
- Current and satisfactory working with children check
