The role involves overseeing case loads of clients who have been discharged from treatment, providing high quality intensive support and case management services. This includes assessment and intervention for high risk clients in the community, preparing clients for court, and engaging in legal processes.
This position is located at the Community Forensic Mental Health Service, 15 minutes from Melbourne's city centre.
Key aspects of the role include:
- Clinical Assessment and Intervention: Conduct thorough assessments of clients' mental health needs and develop tailored intervention strategies.
- Case Management: Provide high-quality case management services to ensure clients receive the necessary support and resources.
- Community Engagement: Engage with clients and their carers to understand their unique needs and develop personalized plans.
- Liaison with Legal System: Work closely with the legal system to prepare clients for court and engage in legal processes.
