As a Support Coordinator, you will assist participants to work towards identified goals by navigating the NDIS service system and ensuring they have access to necessary resources and support.

Key Responsibilities
  • Form ongoing relationships with participants through listening, building rapport, and trust
  • Ensure participants are aware of their rights and can advocate for what they need and want
  • Liaise with internal and external services to maximize participant outcomes
  • Support individuals to manage their own supports
Required Skills and Qualifications
  • Minimum Certificate IV in Mental Health Peer Work or equivalent tertiary qualification; or two years' experience in disability and mental health related work
  • Experience and skills to work competently alongside NDIS participants, families, and support networks
